Bella Terra Schaumburg’s Cardiac Program has been designed to improve the quality of life for patients with heart, lung, and vascular disease.
Highlights of Our Program Include:
Cardiology physicians to manage the program and consult on cardiac visits
Cardiac beds and chairs available to help patients recover more quickly and to promote venous return to major organs
Case management and discharge planning services
Expert staff, including 7-days-a-week support from nurse practitioners to oversee cardiac patients and coordinate efforts with patients’ primary physician, cardiologist, and services
Initial and ongoing assessments from therapists as needed
Medication management
Nutritional services with an on-staff registered dietitian.
On-site diagnostics, including labs, EKG, and chest X-ray services
Personalized education for patients and family members
Physical, occupational and speech therapy up to 7-days-a-week
